This role is pivotal in delivering personalized, comprehensive healthcare to seniors with complex medical needs, fostering an environment that prioritizes proactive, preventative care. As part of our multidisciplinary team, you will have access to state-of-the-art technology and support resources designed to optimize patient outcomes and physician satisfaction. Provide comprehensive primary care in accordance with evidence-based guidelines. Coordinate with multidisciplinary team members to plan and implement effective patient care strategies. Participate in continuous quality improvement initiatives to enhance patient outcomes and service quality. Engage in patient and family education to promote wellness and disease prevention. Lead and mentor healthcare staff, including nurse practitioners, nurses, and other physician extenders. Strong clinical skills in primary care and internal medicine. Excellent communication and interpersonal skills. Proficiency in electronic medical record (EMR) systems. Ability to lead and work collaboratively in a team-oriented environment. Experience in geriatrics or managing complex patient populations is highly desirable.
